It's the "refining" stage I have difficulty with. I also have trouble with the shading itself, trying to determine where the light source is.

I learned light/shadows doing a lot of realistic drawings. It's a matter of experience, I think. I've already seen some tutorials showing how to simplify shapes in a way you can determine easily how shadows will behave. This one can be useful for you:

About the refining, there's a lot of techniques, depending on the artist. I developed my own techniques based on Linda's tutorials:
